Found this to be caused by our Powerloader being setup wrongly for BackupExec. The Powerloader, by default, is set to have the tape drive at a LOWER SCSI ID than the changer (ID 4 - ID6 respectively). Follow Overland Storage Support Document #4151 to fix this problem.
See ME248800 to fix this problem.
Updating the BIOS with the ROMPaq and reinstall the autoloader drivers, seems to have resolved the issue.
