In my case, this error occurs when I open "IBM IWPM Image Plus" for Win XP.
I have seen this error message in connection with Back Again Server 3.0 and 4.0, using a Sony DAT autoloader drive. It seems to happen when the tape is either damaged or old or needs to be re-tensioned. Cleaning the drive with a cleaning tape and re-tensioning the tape has eliminated the error so far.
