In the most basic sense, this is an error stating that the timing is out of sync for some unknown reason. Literally - the two processes of input and output have executed at the same time and crashed into each other; meaning they have overlapped when they should have played nice and taken their turn. When this I/O process is running smoothly, they each take a turn at a regular interval occurring one after the other. One possible way this can occur is that either the input or the output was stalled for some reason and took too much time, and then bad programming in the application compounded the problem by allowing the other process to continue (out of sync) before the first process had finished.
