The first time the problem occurred, it appeared to be either the SCSI cable or the controller card. The controller card and cable were replaced, however, the error continued. The firmware on the card, an Adaptec 2940U2W, was updated to the latest version and the latest drivers were installed as well, but the error still persisted. The problem was eventually tracked to a bad terminator on the end of the cable. After replacing the terminator, the error went away.
