Build a great reporting interface using Splunk, one of the leaders in the Security Information and Event Management (SIEM) field, linking the collected Windows events to www.eventid.net. The EventId.Net for Splunk Add-on assumes that Splunk is collecting information from Windows servers and workstation via the Splunk Universal Forwarder.
The driver beneath this one has failed in some way for \Device\Rdac\database\B\P4P0I0\0.

I had the same problem when I had installed RDAC only with one Emulex Light Pulse PCI Fibre Channel HBA in the machine. After installing the second adapter to the machine I no longer had error 18 in the event log.
As far as I know RDAC is only needed for failover (without you can see the harddisk two times). So if you not have two adapters remove the rdac software.
This is an failure of the IBM-RDAC-Driver, the failover-driver for SAN-Disk´s (most probably for the IBM-FastT-Series).
