As per Emulex KB 13333 (search for the document on the Emulex Knowledgebase), resolution to this error is: "Verify that you have the latest firmware installed on the HBA. If updating the firmware or the latest firmware is already installed and it does not correct the issue, then this set of symptoms indicates the HBA has failed”.
This error occured on a system using Emulex Fiber HBA's to connect to a SAN. After calling their technical support, they confirmed that the hardware should be replaced. It was not a driver issue.
