Array controller P410i [Embedded] has reported an uncorrectable read error during surface analysis operations for logical drive 1. A media error was encountered that is not correctable due to media errors on other physical drive(s) belonging to this logical volume. The uncorrectable media defects are between logical block address 6458880 and logical block address 6459135. The host will be unable to read some blocks between this address range until the blocks are overwritten. Capacity expansion operations must be avoided while the blocks are unreadable.

From an HP support forum: "Replacing the P410i controller cache module seems to have done the trick in my case."
Several hardware support forum suggest that the source of the problem are issues with the hardware itself (controllers, disks, etc). Updating the firmware does not seem to fix this type of problem. Some users suggest enabling and verifying additional logs for more details (logs specific to each hardware platforms, if applicable).
