The problem usually occurs because of corrupted or improperly installed PCI Win modems. Uninstall the modem driver, reseat the modem in a different slot, and reinstall the driver. In one case, the errors started appearing after the modem was removed. I noticed that the driver was not uninstalled and every time the system was restarted, it was looking for the card. When I uninstalled the driver, the problem went away.
