One of the network cables was bad. In our case, we found that the server was sporadically returning pings. When we unplugged one of the cables from the team it started returning pings 100%. Try a "ping <server ip> -t" and unplug one of the cables to see if the server comes back up. I just plugged it into another port and it was fine. This was on a Proliant ML350G4.
