In my case I had URLScan installed, which was blocking certain extensions from running correctly (namely .soap and .rem). The fix was to add the above extensions to the [AllowExtensions] section. Please see ME815155 for a more detailed explanation.

This problem may occur in a large-scale, non-English-language deployment in which the WSUS database grows to more than 2 gigabytes (GB), because the DisableThrottle setting is not included in the installation parameters for some localized versions of WSUS. See ME909456 to solve this problem.
WSUS must have port 80 (HTTP) and port 443 (HTTPS) free to access the Internet in order to synchronise. In our case, this issue was caused by ISA firewall.
