Comment: We have two NICs. External (213.*.*.* / company.co.uk) with OMA and Internal (192.*.*.* /  company.local) with our Intranet. To fix this problem, on the properties for the internal site I added "company.local" to the "Host Header Value" Index: 3582

Comment: In our case the problem was an incorrect attribute value in the Active Directory user object. The attribute 'homeMTA' referenced a deleted value, worked fine when modified the attribute to point to the correct MTA.
The incorrect value was of the format:
CN=Microsoft MTA\0ADEL:e3d3cac8-8e09-4b71-aa5f-7c98cb3fa25f,CN=Deleted Objects,CN=Configuration,DC=[level2],DC=[level1]

The correct format being:
CN=Microsoft MTA,CN=SERVER,CN=Servers,CN=First Administrative Group,CN=Administrative Groups,CN=ORG,CN=Microsoft Exchange,CN=Services,CN=Configuration,DC=[level2],DC=[level1]

The Exchange server had undergone a 'swing migration' which maybe where the problem occurred.
